Developing reports indicate that Precise Benefits Group, doing business as Vault Strategies, may have experienced a data breach that could have exposed personal and medical information.
First detected by Ransomware.Live as having occurred in December 2025, the Vault Strategies data breach was attributed to the cybercriminal group Incransom, with a posted screenshot serving as apparent proof.
On March 18, 2026, DataBreach.com posted a searchable database indexing the reported Vault data breach, which it claims allows individuals to check whether their data was compromised. The breach reportedly involves 129,509 rows of data containing Social Security numbers, dates of birth, email addresses, phone numbers, names, and addresses.
Vault Strategies operates nationwide, providing a comprehensive benefits network, healthcare services, payroll, risk management, and workers’ compensation insurance.
