Operation PAR and Boley Centers, providing mental health, substance abuse disorder treatment, and housing services in Florida, alongside digital health company Eleos, are notifying patients and employees of a data breach.
According to a sample notification letter (pictured below), the companies detected unauthorized network access on or about June 10, 2025. An investigation aided by cybersecurity professionals determined that some files may have been accessed or removed without authorization between June 6 and June 10, 2025. A review of the information that may have been impacted concluded that names, Social Security numbers, medical records, and driver’s license information were involved in the Operation PAR, Boley Centers, and Eleos data breach.
