CHCB Security Incident: What Happened?
Community Health Center of Buffalo (CHCB) is notifying the public about a recent data security incident involving patient information.
An online notice states that on April 21, 2026, CHCB detected suspicious activity within its network. The investigation, supported by digital forensic and cybersecurity experts, revealed that an unknown actor accessed the network on April 20 and April 21 of the same year, retrieving files without authorization.
Presently, Community Health Center of Buffalo is reviewing the data involved and plans to send written notices to those potentially affected. Information believed to be impacted includes personal information, such as names, addresses, dates of birth, Social Security numbers, and driver’s licenses. Protected health information that may have been exposed may include prescription information, treatment location and information, medical diagnoses/conditions, lab results, medications, medical record numbers, provider names, patient medical history, and health insurance policy information and identification numbers.
The Community Health Center of Buffalo data breach was reported to the Department of Health and Human Services on June 19, 2026.
