Provident Behavioral Health Security Incident: What Happened?
Provident Behavioral Health, a mental health care organization with locations across St. Louis, has reported a data breach involving the sensitive information of its current and former patients.
According to a notice posted to its website, Provident Behavioral Health detected suspicious activity on its computer systems on April 3, 2026. After an investigation with a specialized cybersecurity firm, the company determined that an unauthorized user had accessed data stored on the affected systems.
Information that may have been impacted during the incident, varying by individual, included names, contact and demographic information, dates of birth, driver's license or state identification numbers, Social Security numbers, medical information and health insurance information.
The healthcare organization has started sending notices to affected individuals, including its current and former patients and former patients of Care and Counseling, which joined Provident Behavioral Health in 2023. Samples of these notices can be seen below.
The Provident Behavioral Health data breach was also reported to the Massachusetts Office of Consumer Affairs and Business Regulation on September 4.
