Community Dental Care Settlement Wraps Up Class Action Lawsuit Over 2024 Cyberattack

Community Dental Care has agreed to a settlement to wrap up a class action lawsuit that alleged the nonprofit dental care provider failed to safeguard confidential information from a December 2024 data breach.

The Community Dental Care class action settlement, which has received preliminary approval from the court, covers all individuals whose personal information was accessed without authorization in the data breach, including all who received notice of the incident.

Court documents state that approximately 130,583 people were affected by the Community Dental Care data breach.

Community Dental settlement class members who submit a timely, valid claim form can receive up to $5,000 for documented out-of-pocket losses incurred between December 20, 2024 and October 19, 2026 because of the data breach. This benefit covers losses from identity theft or fraud and costs for credit reports, credit monitoring, freezing or unfreezing credit, replacement IDs, postage, and more.

Class members must submit proof, such as bank statements or receipts, to receive a documented-loss payment.

In lieu of a documented-loss payment, class members can file a claim form to receive a $50 cash payment, with no proof required.

Lastly, all class members can receive two years of CyEx Medical Shield Complete, which includes medical identity theft insurance. Enrollment codes were sent to class members with the settlement notice postcard, and there is no need to submit a claim form to receive this service.

To submit a Community Dental settlement claim form online, class members can head to this page and enter the login ID and PIN found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete and return by mail to the settlement administrator.

All Community Dental Care settlement claim forms must be submitted online or postmarked by October 19, 2026.

The court will determine whether to grant final approval to the Community Dental Care settlement following a hearing on November 23, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.

The Community Dental Care class action lawsuit alleged that the Minnesota-based nonprofit dental care provider failed to implement reasonable cybersecurity measures to protect private information from a targeted cyberattack in December 2024.  

Court documents state that confidential information that may have been compromised during the data breach included names, health insurance information, dates of birth, medical information and Social Security numbers.
