A $650K settlement resolves a lawsuit that alleged Roseland Community Hospital Association failed to protect patients’ info from a 2024 data breach.
Roseland Community Hospital Association has agreed to a $650,000 class action settlement that offers cash and medical records monitoring to resolve a lawsuit over a June 2024 data breach.

The Roseland Community Hospital Association class action settlement received preliminary approval from the court on August 18, 2026. Eligible for the settlement are all individuals whose private information was potentially compromised in the data incident and to whom Roseland sent an individual notification that they may have been affected by the data breach.
Court documents state that data breach notice letters were sent to approximately 101,354 people whose information may have been impacted in the cyber incident.

Roseland settlement class members who file a timely, valid claim form can receive up to $5,000 for documented out-of-pocket losses incurred due to the data breach. This benefit covers losses from identity theft or fraud and costs for credit repair services, credit monitoring, freezing/unfreezing credit, professional fees, long-distance phone charges, and more.
Class members must submit proof, such as receipts or credit card statements, to receive a documented-loss payment.
In addition, class members can submit a claim form to receive an estimated $50 cash payment, with no proof required.
The final amount of each class member’s payment may be adjusted on a pro rata (proportional) basis depending on the total number of valid claims filed, among other factors.
Lastly, class members can elect on their claim form to receive one year of medical records monitoring through CyEx Medical Shield Complete or an equivalent service, which includes identity theft insurance. Enrollment codes for the service will be sent to class members after the court grants final approval to the data breach settlement.
To file a Roseland Community Hospital Association settlement claim form online, class members can head to this page and enter the class member ID found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete and return by mail to the settlement administrator.
All Roseland Community Hospital Association settlement claim forms must be submitted online or postmarked by December 31, 2026.
The court will determine whether to grant final approval to the Roseland Community Hospital Association data breach settlement following a hearing on January 6, 2027. Settlement benefits will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.
The Roseland Community Hospital Association class action lawsuit alleged that the Chicago-based healthcare services provider failed to implement adequate cybersecurity safeguards to protect patients’ private information from a data breach that occurred on or around June 2, 2024.
Court documents state that confidential information that may have been compromised in the data incident included names, addresses, dates of birth, diagnoses, lab results, medications, treatment information, health insurance information, provider names, dates of treatment, Social Security numbers and driver’s license numbers.
