McKenzie Health System has agreed to a settlement to end a class action lawsuit that alleged the hospital and healthcare system failed to protect current and former patients’ private information from an April 2025 data breach.

The McKenzie Health System class action settlement received preliminary approval from the court on May 26, 2026. The deal covers all living individuals in the United States whose private information was potentially compromised as a result of the April 2025 data breach, including all who received notice of the incident from McKenzie Health System.

McKenzie settlement class members who file a valid, timely claim form can receive up to $4,000 for out-of-pocket losses incurred between April 14, 2025 and August 24, 2026 due to the data breach. This benefit covers losses resulting from fraud or identity theft and the costs of credit reports, credit monitoring, freezing or unfreezing credit, replacement IDs, postage, and more.
Class members must submit proof, such as bank statements or receipts, to receive reimbursement for out-of-pocket expenses.
In lieu of an out-of-pocket loss payment, class members can instead file a claim form to receive a $50 alternative cash payment, with no proof required.
Finally, in addition to either monetary relief option, all class members can file a claim to receive two years of CyEx Financial Shield Complete, which includes one-bureau credit monitoring, identity theft protection, and financial fraud insurance.
To file a McKenzie settlement claim form online, class members can head to this page and enter the login ID and PIN listed on their copy of the settlement notice. Alternatively, class members can download a PDF of the claim form to print, fill out, and return by mail to the settlement administrator.
All McKenzie Health System settlement claim forms must be submitted online or by mail by August 24, 2026.
The court will determine whether to grant final approval to the McKenzie Health class action settlement following a hearing on October 6, 2026. Compensation will begin to be distributed to class members only after final approval is granted and any appeals are resolved.
The McKenzie Health System class action lawsuit alleged that the Michigan-based healthcare system failed to implement reasonable cybersecurity measures to protect the sensitive information of current and former patients, which allegedly led to a data breach on or around April 14, 2025.
Per court documents, private information potentially exposed during the breach included names, addresses, dates of birth, Social Security numbers, medical record and patient account numbers, diagnoses and treatment information, contact information and demographic information.
