Continuum Health Alliance has agreed to a settlement offering cash and credit monitoring to resolve a class action lawsuit that alleged the healthcare provider failed to adequately protect private patient information from an October 2023 data breach.

The Continuum Health class action settlement received preliminary approval from the court on November 12, 2025 and covers any individuals in the United States whose private information may have been compromised in the October 2023 data breach. Court documents estimate approximately 380,000 people are covered by the settlement.

According to the website, Continuum settlement class members who submit a timely, valid claim form are eligible to receive multiple benefits. Per settlement documents, class members who submit with their claim form documented proof of out-of-pocket losses are eligible to receive a one-time cash payment of up to $5,000, referred to as “Cash Payment A” on all court documents.
In lieu of documented out-of-pocket expenses, class members may instead elect to receive an alternative one-time cash payment of approximately $75, also called “Cash Payment B” on all court documents. Importantly, the settlement site notes that this cash payment is mutually exclusive with reimbursement for documented losses, meaning class members may only submit a claim for one payout.
In addition to monetary benefits, class members are also eligible to enroll in two free years of CyEx Medical Data Monitoring, which includes one-bureau credit monitoring, dark web scanning, victim assistance, identity theft insurance, and access to fraud resolution agents.
As part of the settlement, Continuum Health has also agreed to implement certain business practice changes to better protect the private information stored on its systems and prevent future data security incidents.
To submit a Continuum Health claim form online, class members can head to this page and enter the unique ID found on their copy of the settlement notice. Alternatively, class members can download a PDF of the claim form here, complete it, and return it by mail to the address of the settlement administrator listed near the top of the document.
All Continuum Health settlement claim forms must be submitted online or postmarked by March 2, 2026.
The court will determine whether to grant final approval to the Continuum Health class action settlement at a hearing on March 16, 2026. Compensation will begin to be distributed only after final approval is granted and any appeals are resolved.
The Continuum Health class action lawsuit alleged that the health management provider negligently failed to take measures to prevent an October 2023 data breach that compromised personal information including full names, dates of birth, Social Security numbers, and addresses in violation of the Health Insurance Portability and Accountability Act (HIPAA).
