$950K Coleman Professional Services Settlement Resolves Lawsuit Over 2023 Data Breach

Coleman Professional Services has agreed to a $950,000 settlement to resolve a class action lawsuit that alleged the medical provider failed to safeguard private patient and employee information from a September-October 2023 data breach.

The Coleman Professional Services class action settlement received preliminary approval from the court on May 22, 2026. Eligible to participate in the settlement are all United States residents whose private information was implicated in the data breach, including all to whom Coleman mailed direct notice of the cyber incident.

Court documents state that approximately 51,889 people are included in the settlement class.

Coleman Professional Services class members who submit a timely, valid claim form can receive up to $5,000 for documented out-of-pocket losses incurred due to the data breach. This benefit covers losses from identity theft, fraud or falsified tax returns and costs for credit repair services, freezing/unfreezing credit, credit monitoring, postage, long-distance phone charges, and more.

Class members must submit proof, such as bank statements or receipts, to receive a documented-loss payment.

Additionally, all class members can file a claim form to receive an estimated $70 cash payment, with no proof required.

The final amount of each class member’s cash payment may increase or decrease depending on the total number of valid claims filed, among other factors.

Further, all class members can elect on their claim form to receive one year of three-bureau credit monitoring and identity theft protection services.

To submit a Coleman Professional Services settlement claim form online, class members can head to this page and enter their last name and the claimant ID found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete, and return by mail to the settlement administrator.

All Coleman settlement claim forms must be submitted online or postmarked by October 5, 2026.

The court will decide whether to grant final approval to the Coleman settlement following a hearing on October 13, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.

The Coleman Professional Services class action lawsuit alleged that the Ohio-based behavioral healthcare provider failed to implement reasonable cybersecurity measures to protect sensitive patient information from a data breach that occurred between September 18 and October 31, 2023.

Per court documents, private information that may have been compromised during the breach included names, dates of birth, Social Security numbers, driver’s license or state-issued ID numbers, financial account information, treatment/diagnosis information, patient identification numbers, and health insurance/payment information.
