$2.1M+ OnePoint Patient Care Settlement Ends Class Action Lawsuit Over 2024 Data Breach

OnePoint Patient Care has agreed to a $2,115,000 settlement to conclude a class action lawsuit that alleged the hospice pharmacy services provider failed to protect private information in its care from an August 2024 data breach.

The more than $2.1 million OnePoint Patient Care class action settlement received preliminary approval from the court on May 28, 2026. The deal covers all living United States residents whose private information may have been accessed during the data breach, including all who were sent notice by OnePoint that their information may have been impacted in the cyber incident.

Court documents state that approximately 528,452 people are potentially included in the settlement class.

OnePoint Patient Care settlement class members who submit a timely, valid claim form can receive up to $3,500 for documented out-of-pocket losses related to the data breach. This benefit covers costs for credit reports, credit monitoring, identity theft insurance, bank fees, long-distance phone charges, certain cell phone and data charges, postage, gasoline for local travel, and more.

Class members must submit proof, such as emails or receipts, to receive a documented-loss payment.

In lieu of a documented-loss payment, class members can file a claim form to receive an estimated $100 pro rata cash payment, with no proof required.

The final amount of each class member’s cash payout may be increased or decreased, depending on the total number of valid claims filed, among other factors. 

To submit a OnePoint Patient Care settlement claim form online, class members can head to this page and enter the unique ID and PIN found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete, and return by mail to the settlement administrator.

All OnePoint Patient Care claim forms must be submitted online or postmarked by October 8, 2026.

The court will determine whether to grant final approval to the OnePoint settlement following a hearing on September 23, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.

The OnePoint Patient Care class action lawsuit alleged that the hospice pharmacy and pharmacy benefits manager failed to implement reasonable cybersecurity measures to protect sensitive information from a data breach on or around August 8, 2024.

Court documents state that private information that may have been compromised during the breach included names, addresses, facility locations, and medical information.
