The court-authorized website for the PharMerica data breach class action settlement is live and can be found at PMCSettlement.com.
To submit a claim form online, PharMerica settlement class members can head to this page and enter the class member ID found on their received copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete and return by mail to the settlement administrator listed on the last page of the document.
All PharMerica claim forms must be submitted online or postmarked no later than April 27, 2026.
Individuals who believe they are included in the class but did not receive notice may contact the settlement administrator for more information.

PharMerica is set to pay $5,275,000 to resolve a class action lawsuit over a massive March 2023 data breach that allegedly compromised 4.7 terabytes worth of personal information of millions of individuals in senior living and long-term care services.

The PharMerica data breach settlement received preliminary court approval on January 12, 2026 and covers all living individuals in the United States whose information may have been compromised in the PharMerica data breach and received notice of the breach directly from the pharmacy services provider.

Settlement documents state that PharMerica settlement class members who submit a timely, valid claim form—which will be made available on the forthcoming settlement website—will have multiple options for reimbursement.
Those who submit with their claim form documented proof of out-of-pocket expenses incurred due to the data breach will reportedly be eligible to receive a one-time cash payment of up to $10,000, the agreement outlines. This reimbursement, also called “Cash Payment A” on all court documents, will cover losses that resulted from the March 2025 data breach, including those related to fraud, identity theft, professional fees, credit repair services, and credit monitoring costs.
The settlement agreement explains that class members may also file a claim to receive, in addition to Cash Payment A, a one-time, pro-rated cash payment, or “Cash Payment B,” which will be an equal share of what remains in the net settlement fund after the payment of attorneys' fees, settlement administration expenses, lead plaintiff service awards, and the cost of all other settlement benefits.
In addition to any cash payout, court documents relay that class members can also enroll in one free year of credit monitoring services, which includes dark web monitoring, payday loan notifications, fraud consultation, and identity theft resolution services, without the need to file a claim form.
Lastly, settlement documents indicate that PharMerica has changed its business practices and implemented enhanced security measures to mitigate the risk of a future data breach.

All PharMerica settlement claim forms must be submitted online or postmarked by April 27, 2026.
A hearing to determine whether the court will grant final approval to the PharMerica settlement will be held on May 12, 2026. Compensation will begin to be distributed only after final approval has been granted and any appeals have been resolved.
The PharMerica data breach class action lawsuit alleged that cybercriminals attacked the long term-care pharmacy service provider in March 2023 and obtained a wealth of patients’ personal information. Information compromised in the data breach reportedly included names, dates of birth, Social Security numbers, addresses, medical information, and health insurance information.
