Centrelake Medical Group Settlement Resolves Class Action Lawsuit Over 2019 Data Breach

A Centrelake Medical Group settlement offers cash and credit monitoring to those who may have been affected by an early 2019 data breach.
Centrelake Medical Group has agreed to a settlement offering cash and credit monitoring to resolve a class action lawsuit over a January to February 2019 data breach, during which an unauthorized third party allegedly accessed patients' private health information.

The Centrelake Medical Group class action settlement received preliminary court approval on October 30, 2025, and covers approximately 189,900 Centrelake patients with a California address who received a data breach notice from Centrelake on or around April 16, 2019.

Per the settlement website, Centrelake settlement class members who submit a timely, valid claim form have multiple options for reimbursement.
Class members who submit their claim form with documented proof of ordinary out-of-pocket losses are eligible to receive a one-time cash payment of up to $500 to reimburse expenses incurred as a result of the 2019 data breach, the site relays. Reimbursable expenses, the settlement agreement notes, include those related to bank fees, card reissuance fees, overdraft fees, and late fees. Class members may also file a claim under this reimbursement option for a maximum of three hours of lost time spent remedying issues from the data breach at a rate of $20 per hour, which the site explains is included in the $500 cap to this benefit.
Moreover, the settlement site says that class members who submit documented proof of extraordinary losses related to the data breach are eligible to receive a one-time cash payment of up to $3,500. Per the settlement agreement, covered expenses under this category must also stem from the data breach and include unreimbursed losses due to identity theft or fraud, as well as reimbursement for the repurchase of a medical service at an outpatient radiology service within one year of the breach initially provided by Centrelake but was lost during the data incident.
Additionally, settlement class members who submit proof that they were California residents at the time of the data breach are eligible to receive an additional one-time cash payment of $50.
In addition to any cash payout options, all Centrelake Medical Group settlement class will automatically receive enrollment codes for two free years of CyEx Medical Shield Complete, which provides credit monitoring, medical identity monitoring, and identity theft insurance. According to the settlement website, class members do not need to file a claim to receive this benefit, as enrollment codes have been automatically issued by the settlement administrator

Alternatively, class members can download a PDF of the claim form here to print, complete, and return by mail to the settlement administrator.

All Centrelake settlement claim forms must be submitted online or postmarked by June 12, 2026.
A hearing to determine whether the court will grant final approval to the Centrelake settlement is scheduled for July 14, 2026. Compensation will only be distributed after final approval is granted and any appeals are resolved.
The Centrelake class action lawsuit alleged that the outpatient radiology provider failed to protect patient information stored on its servers, resulting in a data breach where an unauthorized third party accessed and encrypted sensitive patient information y between January 9, 2019 and February 19, 2019.
