Total Vision has agreed to a $475,000 settlement to wrap up a class action lawsuit that alleged the eye care provider failed to protect the sensitive information of patients and others from an October 2020 data breach.

The $475,000 Total Vision class action settlement received preliminary court approval on June 5, 2026. The deal covers all California residents to whom Total Vision sent a direct mail notice of a data breach that occurred on October 30, 2020.
Court documents estimate that 88,722 people are covered by the class action settlement.

Total Vision settlement class members who file a timely, valid claim form can receive up to $1,000 for documented out-of-pocket losses traceable to the data breach. This benefit covers losses caused by identity theft or fraud and costs for credit reports, credit monitoring, credit freezes, replacement IDs, bank fees, and more.
Class members must submit proof, such as bank statements or receipts, to receive a documented-loss payment.
Additionally, class members can submit a claim form to receive a pro rata cash payment from the class action settlement fund, with no proof required.
All claims for documented-loss payouts must be printed from the settlement website, completed, and submitted by mail. Claim forms for documented losses will not be accepted online.
Class members who wish to file a claim form for a pro rata cash payment may file their claim online.
To submit a Total Vision settlement claim form online, class members can head to this page and enter their last name and the ILYM ID and claim ID found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete, and return by mail to the settlement administrator.
All Total Vision settlement claim forms must be submitted online or postmarked by October 5, 2026.
The court will determine whether to grant the Total Vision settlement final approval following a hearing on December 18, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.
The Total Vision class action lawsuit alleged that the California-based eye care provider failed to implement reasonable cybersecurity measures to protect sensitive information pertaining to patients and others from a data breach on October 30, 2020.
Per court documents, confidential information that may have been compromised in the breach included names, addresses, dates of birth, Social Security numbers, and prescription information.
