Mt. Baker Imaging, LLC and Northwest Radiologists, Inc. have agreed to a $3,300,000 settlement to wrap up a class action lawsuit that alleged the medical imaging provider failed to safeguard confidential information from a January 2025 data breach.

The $3.3 million Mt. Baker Imaging and Northwest Radiologists class action settlement received preliminary approval from the court on April 21, 2026. The agreement covers all living United States residents whose private information was potentially or actually compromised in the data breach that occurred between January 20 and January 25, 2025.
Court documents indicate that approximately 340,184 people are covered by the data breach class action settlement.

Mt. Baker settlement class members who submit a timely, valid claim form can receive up to $5,000 for documented out-of-pocket losses incurred as a result of the data breach. This benefit covers losses stemming from identity theft or fraud and costs for credit monitoring, bank fees, postage, notary charges, and more.
Class members must submit proof to receive an out-of-pocket-loss payment.
Additionally, all class members can submit a claim form to receive a pro rata cash payment, with no proof required, from the amount that remains in the settlement fund after all other benefits and expenses have been paid.
Finally, all class members can submit a claim form to receive two years of CyEx Medical Shield Complete, which includes healthcare data monitoring and protection.
To submit a Mt. Baker settlement claim form online, class members can head to this page and enter the settlement claim ID found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete, and return by mail to the settlement administrator.
All Mt. Baker and Northwest Radiologists settlement claim forms must be submitted online or postmarked no later than August 19, 2026.
The court will determine whether to grant the Mt. Baker Imaging and Northwest Radiologists settlement final approval following a hearing on August 21, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.
The Mt. Baker Imaging and Northwest Radiologists class action lawsuit alleged that the Washington-based medical imaging provider failed to implement appropriate cybersecurity safeguards to prevent a January 2025 ransomware attack, leading to a multi-day data breach.
According to court documents, confidential information that may have been compromised in the breach included protected health information and personally identifiable information.
