$2.5M Esse Health Settlement Wraps Up Class Action Lawsuit Over April 2025 Data Breach

A $2.5M Esse Health settlement offers cash and medical data monitoring to individuals potentially affected by an April 2025 data breach.

American Multispecialty Group Inc., which does business as Esse Health, has agreed to a $2,525,000 settlement to resolve a class action lawsuit that alleged the healthcare group failed to protect confidential information in its network from an April 2025 data breach.

The $2.5 million Esse Health class action settlement received preliminary approval from the court on April 9, 2026. The settlement covers all individuals who were potentially impacted by the data breach, including those who were sent notice by Esse Health stating that their private information may have been compromised.
Court documents state that approximately 521,167 people are covered by the settlement.

Esse Health class members who submit a timely, valid claim form can receive an approximately $50 cash payment with no proof required. Court documents note that the final amount of this cash payment may increase or decrease, depending on the total number of claims filed.
To submit an Esse Health settlement claim form online, class members can head to this page and enter the login ID and PIN found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, fill out and return by mail to the settlement administrator.
All Esse Health settlement claim forms must be submitted online or postmarked by August 4, 2026.
Additionally, all class members are automatically eligible to receive two years of CyEx Medical Shield Complete, which includes medical identity theft insurance.
To enroll in this service, class members must head to this page and enter the code also found on their copy of the settlement notice, along with their email address and first name.
The court will determine whether to grant the Esse Health settlement final approval following a hearing on August 3, 2026. Cash payments will be sent out and medical identity protection subscriptions will become active only after final approval is granted and any appeals are resolved.
The Esse Health class action lawsuit claimed that the Missouri-based physician group failed to implement reasonable cybersecurity safeguards to protect the sensitive information in its care, which allegedly led to a data breach in April 2025.
Per court documents, private information potentially exposed during the breach includes names, addresses, dates of birth, health insurance information, patient ID numbers, certain health information and the Social Security numbers of some individuals.
