A Triage Staffing settlement offers cash and credit monitoring to individuals potentially impacted by a May 2025 data breach.
Triage LLC, which does business as Triage Staffing, has agreed to a settlement to wrap up a class action lawsuit that alleged the healthcare staffing company failed to protect sensitive information in its systems from a May 2025 data breach.

The Triage Staffing class action settlement received preliminary approval from the court on April 10, 2026. The settlement covers all United States residents whose personal information was potentially compromised in the May 2025 data breach and who were sent notice of the incident.

Triage settlement class members who file a valid, timely claim form can receive up to $500 for documented ordinary losses incurred between May 2025 and August 10, 2026 stemming from the data breach. Losses covered by this benefit include the cost of professional fees, credit repair services, credit monitoring, freezing or unfreezing credit, replacement IDs and more.
Additionally, class members can file a claim form to receive up to $2,500 for documented extraordinary losses incurred between May 2025 and August 10, 2026. This benefit covers expenses related to identity theft or fraud arising from the data breach that are not already covered by the ordinary-loss benefit.
Class members must submit proof, such as receipts, to receive these benefits.
In addition to an ordinary- and/or extraordinary-loss payment, class members can file a claim form to receive two years of CyEx Identity Defense Total, which includes three-bureau credit monitoring and identity protection insurance.
In lieu of any other benefits, class members can file a claim to receive an alternative $50 cash payment with no proof required.
To file a Triage settlement claim form online, class members can head to this page and log in using the unique ID and PIN listed on their copy of the settlement notice. Alternatively, class members can download a PDF of the claim form to print, fill out, and either send an email with an image of the completed claim form or return it by mail to the settlement administrator.
All Triage settlement claim forms must be submitted online, emailed or postmarked by August 10, 2026.
The court will determine whether to grant final approval to the Triage Staffing settlement following a hearing on August 18, 2026. Compensation will begin to be distributed to class members only after final approval is granted and any appeals are resolved.
The Triage Staffing class action lawsuit alleged that the medical staffing and travel nursing agency failed to implement proper cybersecurity measures to safeguard the sensitive information in its care, which led to a data breach Triage became aware of on or around May 1, 2025. Per court documents, private information that may have been exposed during the breach includes full names, addresses, dates of birth, and Social Security numbers.
