Summit Medical Group Settlement Resolves Lawsuit Over 2024 Data Breach

Summit Medical Group has agreed to a class action settlement to resolve a lawsuit over the Tennessee healthcare network’s alleged failure to protect current and former patients’ and employees’ confidential information from a September 2024 data breach.

The Summit Medical Group class action settlement received preliminary approval from the court on July 28, 2026. Eligible to participate in the settlement are all living individuals in the United States whose private information was potentially compromised in the data breach, including all who were mailed notice of the incident by Summit Medical Group.

Court documents state that there are approximately 464,000 settlement class members.

Summit Medical class members who submit a timely, valid claim form can receive up to $2,500 for documented out-of-pocket losses incurred between September 2024 and November 4, 2026 due to the data breach. This benefit covers losses from identity theft or fraud and costs for credit reports, credit monitoring, freezing/unfreezing credit, replacement IDs, postage, and more.

Class members must submit proof, such as receipts, to receive a documented-loss payment.

In addition, all class members can file a claim form to receive reimbursement for up to three hours of lost time spent addressing the data breach, at a rate of $15 per hour. This benefit covers time spent changing passwords, investigating suspicious account activity, and researching the data incident.

Should the total amount of all documented-loss and lost-time payments exceed $500,000, payments to class members will be reduced on a pro rata basis.

Lastly, all class members can elect on their claim form to receive two years of CyEx Medical Shield Complete, which includes medical data monitoring and medical identity theft insurance.

To submit a Summit Medical Group settlement claim form online, class members can head to this page and enter the login ID and PIN found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete and return by mail to the settlement administrator.

All Summit Medical settlement claim forms must be submitted online or postmarked by November 4, 2026.

The court will determine whether to grant final approval to the Summit Medical settlement following a hearing on November 19, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.

The Summit Medical Group class action lawsuit alleged that the Tennessee-based medical provider network failed to implement reasonable cybersecurity measures to protect confidential information belonging to current and former patients and employees from a September 2024 data breach.

Court documents state that information that may have been compromised in the breach included names, provider names, dates of service, which facility they visited, treatment and prescription information, health insurance information, and Social Security numbers.
