A Naper Grove Vision Care settlement offers cash and medical monitoring to those whose information may have been impacted by a May 2025 data breach.
Naper Grove Vision Care has signed on to a class action settlement to resolve a lawsuit that alleged the optometry group failed to protect confidential patient information in its care from a May 2025 data breach.

The Naper Grove Vision Care class action settlement received preliminary approval from the court on May 19, 2026. The deal covers all individuals whose private information may have been impacted by the Naper Grove data breach.
Court documents state that approximately 20,093 people are covered by the data breach settlement.

Naper Grove settlement class members who submit a timely, valid claim form can receive up to $1,000 for documented out-of-pocket losses incurred between May 24, 2025 and September 18, 2026 due to the data breach. This benefit covers losses from identity theft or fraud and costs for credit reports, credit monitoring, freezing and unfreezing credit, replacement IDs, postage, and more.
Class members must submit proof, such as bank statements or receipts, to receive a documented-loss payment.
In lieu of a documented-loss payment, class members can file a claim form to receive a pro rata cash payment, with no proof required.
The amount of each class member’s cash payout will be determined based on the number of valid claims filed.
To submit a Naper Grove settlement claim form online, class members can head to this page and enter the login ID and PIN found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, complete, and return by mail to the settlement administrator.
All Naper Grove claim forms must be submitted online or postmarked no later than September 18, 2026.
Additionally, all class members are eligible to enroll in one year of CyEx Medical Shield Complete, which includes medical record monitoring and medical identity theft insurance. Each class member was sent a CyEx enrollment code on their email or postcard settlement notice.
The court will determine whether to grant the Naper Grove settlement final approval following a hearing on October 20, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.
The Naper Grove Vision Care class action lawsuit alleged that the Naperville, Illinois-based optometry group failed to implement reasonable cybersecurity measures to protect sensitive information pertaining to current and former patients, allegedly leading to a May 2025 data breach.
Per court documents, confidential information that may have been compromised in the breach included names, Social Security numbers, addresses, dates of birth, driver’s license numbers, patient numbers, health insurance numbers, benefits documentation, and medical conditions or treatment information.
