The Medical Associates of Brevard class action settlement received preliminary court approval on September 7, 2025 and covers all living United States residents whose private information may have been impacted in the January 24, 2025 MAB data breach.
The court-approved website for the Medical Associates of Brevard settlement can be found at MedAssociatesBrevardDataBreach.com.
Medical Associates of Brevard settlement class members who submit a timely, valid claim form can receive medical data monitoring services and up to $1,500 in reimbursement for documented, out-of-pocket losses incurred due to the cyberattack.
Per the settlement website, the medical data monitoring includes one year of CyEx Medical Shield Pro, which offers $1 million of medical identity theft insurance and monitoring for medical record number exposure, healthcare insurance ID exposure and unauthorized health savings account spending.
In order to receive reimbursement for data breach-related losses, class members must submit with their claim form reasonable documentation of each loss, such as bank statements or receipts, the website states. If the documents do not clearly show how the loss is related to the data breach, claimants must submit additional notes explaining the connection, the settlement site adds.
Claimable losses must have occurred between January 24, 2025 and December 1, 2025, and may include expenses such as:
- Fees for credit reports, credit monitoring or freezing/unfreezing credit due to theft or fraud;
- Identity theft- or identity fraud-related losses, including ID replacement costs; and
- Postage used to contact banks by mail because of fraud or theft.
To submit a Medical Associates of Brevard claim form online, class members can visit this page of the settlement website and log in with the unique ID and PIN found in their copy of the settlement notice.
Alternatively, a PDF of the claim form is available to print, fill out and mail back to the address listed on the second page of the form.
All class action settlement claim forms must be submitted online or postmarked by December 1, 2025.
A hearing is scheduled for December 16, 2025 to determine whether the Medical Associates of Brevard settlement will receive final approval from the court. Settlement benefits will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.
The data breach class action lawsuit claimed that MAB failed to protect its network from cybercriminals, who were able to illegally access the sensitive personal information of tens of thousands of people on January 24, 2025.
