The Hospital Authority of the City of Bainbridge and Decatur County is set to pay $500,000 to settle a class action lawsuit over a November 2024 data breach that allegedly compromised the private information of patients of Memorial Hospital and Manor.

The Memorial Hospital and Manor class action settlement received preliminary approval from the court on October 21, 2025 and covers approximately 105,000 current and former Memorial Hospital and Manor patients residing in the United States who received notice that their private information may have been accessed during the data breach.

According to the settlement website, Memorial Hospital settlement class members who submit a valid, timely claim form have multiple options for reimbursement. Those who submit with their claim form documented proof of out-of-pocket losses stemming from the data breach are eligible to receive a one-time cash payment of up to $5,000. The site adds that data breach-related expenses covered under this reimbursement option must have occurred between November 2, 2024 and January 5, 2026, and include costs related to identity theft, credit monitoring, freezing credit and ID replacement.
In lieu of a payment for documented out-of-pocket losses, class members who spent time remedying issues related to the Memorial Hospital and Manor data breach are eligible to receive reimbursement for up to four hours of lost time, at a rate of $25 per hour, the settlement website states.
Class action settlement class members who do not submit a claim form for documented-loss or lost-time reimbursement are eligible to receive a one-time alternative cash payment of $40. Under this reimbursement option, no proof or explanation is required from a class member to receive this payment.
Per the settlement agreement, class members may elect to receive their payout via cash or electronic payment, and all checks must be cashed within 90 days after issuance before expiration.
In addition to monetary reimbursement, all settlement class members are eligible to enroll in one free year of CyEx Medical Shield, which provides monitoring for health insurance ID numbers, medical records exposure and unauthorized Health Savings Account spending.
To submit a Memorial Hospital and Manor settlement claim form online, class members can head to this page and enter the class member ID and PIN found on their copy of the settlement notice. Consumers who believe that they may be a class member but did not receive a settlement notice should contact the settlement administrator to confirm their identity and receive their login information.
Alternatively, class members can download a PDF of the claim form to print, fill out, and return by mail to the address listed at the bottom of the second page of the document.
All Memorial Hospital claim forms must be submitted online or by mail by January 5, 2026.
The court will determine whether to grant final approval to the Memorial Hospital settlement at a hearing on January 20, 2026. Compensation will begin to be distributed to class members only after final approval is granted and any appeals are resolved.
The Memorial Hospital and Manor class action lawsuit claimed that the Georgia healthcare provider failed to implement adequate security measures to protect private patient information, leading to a cyberattack in November 2024. According to the settlement website, the current and former patient data allegedly impacted during the incident included names, dates of birth, Social Security numbers and private health information.
