According to the settlement website, Alabama Cardiology settlement class members who submit a timely, valid claim form are eligible for several benefits. Class members who submit with their claim form documented proof of out-of-pocket losses are eligible to receive a one-time cash payment of up to $5,000. These documented losses, the agreement adds, must have been reasonably incurred as a result of the July 2024 data breach, and reimbursable expenses include those related to credit monitoring, bank fees, fraud, unauthorized charges, and other potential misuse of personal information.
In lieu of documented out-of-pocket expenses, class members may instead elect to receive an alternative one-time cash payment estimated to be $100. Alternative cash payments will be a pro rata (equal share) portion of what remains in the net settlement fund after the payment of all other expenses and benefits, the settlement agreement notes.
In addition to any monetary benefit, the settlement site reports that all settlement class members are also eligible to receive two free years of CyEx Medical Shield Complete, which includes services such as one-bureau credit monitoring and up to $1,000,000 in identityand identity theft insurance. Per the agreement, CyEx activation codes will be valid for 12 months after issuance to class members.
To submit an Alabama Cardiology settlement claim form online, class members can head to this page of the settlement site and enter the unique ID found on their copy of the settlement notice. All Alabama Cardiology settlement claim forms must be submitted online or postmarked by March 6, 2026.
The court will determine whether to grant final approval to the Alabama Cardiology settlement at a hearing on March 20, 2026. Compensation will be distributed to consumers only after final approval is granted and any appeals are resolved.
