A $1M Clay-Platte Family Medicine Clinic settlement wraps up a class action lawsuit over a June 2024 data breach.
Clay-Platte Family Medicine Clinic, Summit Family and Sports Medicine, and Barry Pointe Family Care have agreed to a $1,000,000 settlement to resolve a class action lawsuit that alleged the practices failed to safeguard confidential information from a June 2024 data breach.

The Clay-Platte Family Medicine Clinic class action settlement received preliminary approval from the court on June 16, 2026. The deal covers the approximately 53,916 United States residents who were sent notice by the defendants that their personal information or protected health information was potentially compromised because of the data breach.

Clay-Platte settlement class members who file a timely, valid claim form can receive up to $15,000 for documented out-of-pocket losses incurred due to the data breach. This benefit covers losses from identity theft or fraud and costs for credit monitoring services purchased after June 26, 2024, professional fees, credit repair services, postage, mileage, phone charges, and more.
Class members must submit proof, such as receipts or invoices, to receive a documented-loss payment.
In lieu of a documented-loss payment, class members can submit a claim form to receive a pro rata cash payment, with no proof required.
The final amount of this cash payment will be determined based on the total number of valid claims filed, among other factors.
Finally, all class members can elect on their claim form to receive three years of CyEx Medical Shield Total, which includes three-bureau credit monitoring, dark web monitoring, identity theft insurance, and more.
To submit a Clay-Platte Family Medicine Clinic settlement claim form online, class members can head to this page and enter the notice ID and confirmation code found on their copy of the settlement notice. Alternatively, class members can download a PDF claim form to print, fill out, and return by mail to the settlement administrator.
All Clay-Platte settlement claim forms must be submitted online or postmarked by September 30, 2026.
The court will determine whether to grant final approval to the Clay-Platte Family Medicine Clinic settlement following a hearing on September 29, 2026. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.
The Clay-Platte Family Medicine Clinic class action lawsuit alleged that the clinic and other Missouri-based healthcare providers failed to implement reasonable cybersecurity measures to protect patients’ private health information from a data breach on June 26, 2024.
Court documents state that private information that may have been compromised in the breach included names, Social Security numbers, dates of birth, addresses, confidential medical information, genders, and telephone numbers.
