The deal covers all United States residents who were notified by the firm that their private information was or may have been impacted in the Keenan & Associates data breach, which reportedly occurred between August 21 and August 27, 2023.
According to the official Keenan data breach settlement website, eligible class members must submit a timely, valid claim form to receive benefits from the deal, which include three years of complimentary credit monitoring and identity theft protection services and either a pro-rated cash payment or reimbursement for certain out-of-pocket losses.
The deadline to file a Keenan & Associates settlement claim form is October 30, 2025.
Head to this page to submit a claim form online. Alternatively, you can download a PDF claim form to print, complete and return to the settlement administrator by mail.
Claim form submission requires a unique CPT ID, which can be found in the personalized settlement notice you should have received. Filing online requires a passcode that is also provided in the notice. If you did not receive a settlement notice, you may still submit a claim form by mail.
As part of the deal, class members can file a claim form for reimbursement of up to $10,000 per person for documented losses incurred on or after August 21, 2023 that are reasonably traceable to the data breach and have not already been reimbursed through insurance, the settlement agreement says.

In lieu of reimbursement for such losses, consumers can submit a claim for a pro-rated cash payment, the agreement states, noting that California residents will receive twice the amount issued to residents of all other states.
The settlement website says that with respect to the offer of credit monitoring services, class members who already maintain such coverage can choose to defer their enrollment for 12 months for no additional charge.
Per the settlement agreement, the duration of credit monitoring services as well as cash payment amounts are subject to adjustment depending on how many class members submit valid claims.
Keenan has also agreed to implement and maintain enhanced cybersecurity measures in response to the incident, the website relays.
The agreement, which resolves the lawsuit against Keenan & Associates, received preliminary approval from the court on July 3, 2025. Next, the court will decide whether to grant final approval to the terms of the deal at a hearing on November 14, 2025.
Settlement benefits are typically issued to eligible class members only if the deal receives ultimate court approval and after any appeals are resolved.
