A $1.2M Sunflower Medical Group settlement ends a class action lawsuit over a Dec. 2024 data breach that allegedly exposed private patient data.

Sunflower Medical Group has agreed to pay $1.2 million to settle a class action lawsuit over a December 2024 data breach that potentially compromised sensitive patient information and health records.

The Sunflower Medical Group class action settlement received preliminary approval from the court on October 27, 2025 and covers approximately 255,734 United States residents who received notice from Sunflower that an unauthorized third party may have accessed their personal information as a result of the December 2024 data breach.

According to the website, Sunflower settlement class members who submit a timely, valid claim form are eligible for one of two cash payments. Settlement class members who submit documented proof of ordinary out-of-pocket expenses resulting from the data breach are eligible to receive a one-time cash payment of up to $5,000, the settlement site says. Under this reimbursement option, the agreement notes that eligible expenses include those related to identity theft and fraud, fees for credit reports, fees to freeze/unfreeze credit, costs to replace IDs, and small expenses like postage.
In lieu of a documented loss payment, the settlement site relays that class members may instead elect to receive a one-time alternative cash payment of approximately $10. The final amount of the alternative cash payment, the agreement outlines, may become pro-rated depending on the total number of valid claims submitted.
According to the settlement website, all Sunflower class members are eligible to enroll in two free years of CyEx Medical Shield Complete in addition to either cash payout option.
To submit a claim form online, class members can visit this page and enter the unique ID and PIN found on their copy of the settlement notice. Alternatively, class members can download a PDF of the claim form from the settlement site to print, fill out, and return by mail to the address listed on the second page of the claim form.
All Sunflower Medical Group claim forms must be submitted online or by mail by March 26, 2026.
The court will determine whether to grant final approval to the Sunflower settlement at a hearing on March 6, 2026. Compensation will begin to be distributed only after final approval is granted and any appeals are resolved.
The Sunflower Medical Group class action lawsuit alleged that an unauthorized third party gained access to patients’ information during a December 2024 data breach. According to court documents, the compromised information included full names, Social Security numbers, addresses, driver's license numbers, medical records, and health insurance information.
