Precision Imaging Centers is set to pay as much as $200,000 to settle a class action lawsuit over a November 2022 cyberattack that allegedly compromised private patient information.

The Precision Imaging Centers class action settlement received preliminary approval from the court on September 3, 2025, and covers approximately 31,010 individuals who received a notification from the healthcare provider in June 2023 regarding the data breach.

Precision settlement class members who submit a valid, timely claim form have multiple options for reimbursement. Per the settlement website, class members who submit with a valid claim form documented proof of ordinary out-of-pocket expenses and lost time are eligible to receive a one-time cash payment up to $500. Per the settlement website, costs covered under the ordinary-expense reimbursement option include credit card fees, costs to access credit reports, telephone charges and credit monitoring services incurred on or after November 2, 2022.
Class members may also submit a claim for up to four hours of lost time spent remedying issues related to the data breach, at a rate of $20 per hour, the site adds.
Class members who submit documented proof of extraordinary out-of-pocket expenses with a valid claim form are eligible to receive a one-time cash payment up to $5,000. Costs covered under this reimbursement option include those related to identity theft, fraud, falsified tax returns, or other misuse of personal information that occurred on or after November 2, 2022.
According to the settlement site, class members may submit a claim form for reimbursement for up to 8 hours of lost time spent dealing with identity theft, fraud or other misuse of their personal information that’s “fairly traceable” to the data breach, at a rate of $20 per hour.
Per the settlement agreement, Precision class members can claim either or both monetary compensation options, and can elect to receive their payment via check or electronic payment. Checks must be cashed by class members within 90 days of issuance before expiration.
The amount each class member will receive in monetary compensation may be reduced on a pro-rated basis, depending on the number of valid claims submitted and the total costs associated with settlement administration, attorney’s fees, and service awards, the settlement website states.
In addition to monetary reimbursement, class members with valid claim forms are also eligible to receive two free years of identity protection and credit monitoring services.
To submit a Precision Imaging Center settlement claim form online, class members can head to this page and enter the class member ID found on their copy of the settlement notice. Consumers who believe they may be a class member but did not receive a settlement notice can contact the settlement administrator to receive a login ID or generate a unique claim form to print and mail in.
Settlement class members who did receive a notice may also log in to the settlement site and generate a physical claim form with their name, class member ID, and other relevant information to print and mail in.
Precision Imaging Center settlement claim forms must be submitted online or by mail by January 31, 2026.
The court will determine whether to grant final approval to the Precision Imaging Centers settlement at a hearing on January 8, 2026. Compensation will begin to be distributed to consumers only after final approval is granted and any appeals are resolved.
Lastly, Precision Imaging Center, as part of the settlement, has agreed to implement certain data security measures to address the cyberattack and maintain the measures for three years, and to purge or encrypt the Social Security numbers of patients who have not obtained the company’s services for five years or more, the website states.
The Precision Imaging Center class action lawsuit alleged that an “unauthorized consortium of sophisticated threat actors” targeted the Jacksonville, Florida-based healthcare provider’s network in November 2022 and accessed the private data of current and former patients. According to court documents, the patient information impacted by the data breach included, but was not limited to, names, addresses, dates of birth, Social Security numbers, driver’s license and ID numbers, health insurance information and medical histories.
