Pinehurst Radiology Settlement Ends Class Action Lawsuit Over 2025 Data Breach

Pinehurst Radiology Associates, PLLC, has agreed to a class action settlement that includes cash payments and medical data monitoring to end a class action lawsuit that claimed the practice failed to prevent a January 2025 data breach.

The Pinehurst Radiology class action settlement received preliminary court approval on December 11, 2025 and covers all individuals in the United States whose personally identifiable information and/or protected health information was implicated in the January 2025 Pinehurst data breach.

Pinehurst settlement class members who submit a timely, valid claim form can receive one year of CyEx Medical Shield Complete and up to $500 in reimbursement for documented, out-of-pocket losses related to the cyberattack.
Per the settlement website, CyEx Medical Shield Complete offers one year of medical data monitoring services, including $1 million in medical identity theft insurance, access to fraud resolution agents, real-time alerts, victim assistance, and monitoring of the following for suspicious use or activity:
- Medical records;
- National provider identifiers;
- Healthcare insurance plan IDs;
- Health savings accounts;
- Healthcare beneficiary identifiers;
- International classification of diseases; and
- The dark web.
Settlement documents state that losses for which a class member can submit a claim form for reimbursement must have occurred between January 20, 2025 and April 9, 2026, and must be accompanied by reasonable documentation, such as receipts or bank statements. Class members cannot claim losses that have already been reimbursed by another party. Per the settlement website, reimbursable losses include:
- Fees for credit reports, credit monitoring or freezing and unfreezing credit;
- Postage to contact banks by mail;
- Losses because of identity theft or fraud; and
- Costs of replacing IDs.
To submit a Pinehurst Radiology claim form online, class members can visit this page of the settlement website and log in with the unique ID and PIN found in their copy of the settlement notice.
Alternatively, a PDF claim form is available to print, fill out and mail back to the address listed on the second page of the document.
All settlement claim forms must be submitted online or postmarked by April 9, 2026.
A hearing is scheduled for April 6, 2026 to determine whether the class action settlement will receive final approval from the court. Settlement benefits will be distributed only after final approval has been granted and any appeals have been resolved.
The Pinehurst Radiology class action lawsuit claimed that the North Carolina-based practice failed to prevent a data breach on January 20, 2025 that compromised or otherwise exposed sensitive personal and medical data—including Social Security numbers, medical treatment details, health insurance information and Medicare/Medicaid numbers—to an unauthorized third party.
