The Park Dental class action settlement received preliminary approval from the court on September 29, 2026. Eligible for the settlement are all living individuals in the United States whose private information was potentially compromised in the Park Dental data breach.
The court-authorized Park Dental data breach settlement website can be found at ParkDentalDataSecuritySettlement.com.
Settlement class members who file a timely, valid claim form can receive up to $2,500 for unreimbursed, documented out-of-pocket losses incurred due to the data breach. This benefit covers losses from identity theft or fraud and costs for credit reports, credit monitoring services purchased between December 6, 2024 and December 28, 2026, bank fees, long-distance phone calls, postage, and more.
Class members must submit proof, such as receipts or bank statements, to receive a documented-loss payment.
In lieu of a documented-loss payment, class members whose Social Security numbers and driver’s license numbers were impacted by the data incident can file a claim form to receive reimbursement for up to four hours of lost time spent responding to the data breach, at a rate of $15 per hour.
Lastly, class members can elect on their claim form to receive one year of CyEx Financial Shield Complete, which includes credit monitoring and identity theft protection insurance.
To file a Park Dental settlement claim form online, class members can navigate to this page and select whether they have the claim number found on their copy of the settlement notice. Alternatively, class members can, on the same page, select the option indicating that they do not have a claim number to download a personalized claim form to print, complete, and return by mail to the settlement administrator.
All Park Dental settlement claim forms must be submitted online or postmarked by December 28, 2026.
The court will determine whether to grant final approval to the Park Dental class action settlement following a hearing on February 1, 2027. Settlement benefits will begin to be made available to class members only after final approval has been granted and any appeals have been resolved.
The Park Dental class action lawsuit alleged that the network of dental care providers, which has more than 150 dentists at more than 50 locations throughout Minnesota and western Wisconsin, failed to implement adequate cybersecurity measures to safeguard private information from a data breach reportedly discovered by Park Dental on or around January 23, 2024.
Court documents state that personal information that may have been compromised in the data security incident included names, medical information, Social Security numbers, driver’s license numbers, dates of birth, and financial account and payment card information.
