Alabama Ophthalmology Associates, P.C., has agreed to an $850,000 class action settlement to resolve a lawsuit that alleged the medical practice failed to protect the sensitive information of current and former patients and employees from a January 2025 data breach.

The Alabama Ophthalmology Associates class action settlement received preliminary approval from the court on February 19, 2026. The deal covers all United States residents who were notified by the practice that their information may have been impacted during the data breach.
Court documents state that approximately 153,575 people were notified of the breach by Alabama Ophthalmology.
The court-approved website for the Alabama Ophthalmology Associates (AOA) data breach settlement can be found at AOASettlement.com.
According to the website, AOA settlement class members who file a timely, valid claim form can receive up to $5,000 for out-of-pocket losses stemming from the data breach. To receive this benefit, class members must submit documentation, such as receipts.
This documented-loss payment is meant to cover data breach-related expenses incurred due to fraud and identity theft, as well as costs stemming from professional fees, obtaining credit repair services, freezing or unfreezing credit, credit monitoring, and postage and travel.
In lieu of a documented-loss payment, class members may instead file a claim form to receive a cash payment of approximately $60, with no proof required. Court documents say that the final amount of each class member’s payment will depend on the total number of valid claims filed.
In addition to either cash payment option, all settlement class members who file a valid claim form can receive two free years of CyEx Medical Shield Complete. This service includes credit monitoring, health insurance ID and medical record number monitoring, health savings account monitoring, and identity theft insurance.
To file an Alabama Ophthalmology Associates settlement claim form online, class members can head to this page and log in using the unique ID and PIN found on their copy of the settlement notice. Alternatively, class members may download a PDF of the claim form to print, fill out and return by mail to the settlement administrator.
All Alabama Ophthalmology settlement claim forms must be submitted online or by mail by July 6, 2026.
The court will determine whether to grant final approval to the Alabama Ophthalmology Associates data breach settlement following a hearing on June 25, 2026. Compensation will begin to be distributed only after final approval has been granted and any appeals have been resolved.
The Alabama Ophthalmology Associates class action lawsuit claimed that the eye care specialists failed to implement reasonable cybersecurity measures to protect the sensitive information of current and former patients and employees, which led to a data breach on or around January 22, 2025. Per court documents, private information that may have been compromised during the breach included names, Social Security numbers, and health insurance and treatment information.
