$675K Dignity Health Settlement Ends Data Breach Lawsuit Over 2023 R1 RCM Hack

The settlement agreement, which also resolves claims brought against third-party vendor R1 RCM, Inc., received preliminary approval from the court on July 14, 2025. The class action settlement covers all individuals whose personal and/or health information was potentially impacted by the Dignity Health and R1 RCM data breach, including those who were sent notice of the incident.
To receive Dignity Health settlement benefits—that is, reimbursement for certain expenses, credit monitoring services and a cash payment—eligible class members must file a valid Dignity Health settlement claim form online or by mail by November 11, 2025.
Class members can submit a Dignity Health claim form online on this page. Those who wish to file by mail can download a PDF claim form or contact the settlement administrator to request a paper copy.
Claim form submission requires a unique notice ID, which can be found in the personalized settlement notice you should have received. To file a claim form online, consumers will need their PIN, which is also included in the notice.
According to the Dignity Health settlement website, class members who submit a timely, valid claim form are eligible to receive two years of CyEx Medical Shield Total credit monitoring and medical identity theft protection services at no cost.

In addition, eligible consumers can file a claim form to receive reimbursement of up to $500 per person for documented out-of-pocket expenses that were incurred on or after January 30, 2023 and are reasonably traceable to the data breach, the settlement agreement says. Per the agreement, examples of these costs may include charges related to accessing, freezing or unfreezing credit; phone, fax, mileage, notary or postage fees; credit monitoring expenses; and other miscellaneous costs.
Moreover, the website states that class members may file a claim form for reimbursement of up to $2,500 per person for “extraordinary” losses that are fairly traceable to the incident and supported by documentation. According to the class action settlement agreement, this benefit covers unreimbursed expenses linked to identity theft, falsified tax returns or other forms of fraud incurred between January 30, 2023 and the date a claim form is submitted.
The website shares that consumers can also submit a claim form for a pro-rated Dignity Health settlement cash payout. Individual payment amounts will depend on what remains of the $675,000 settlement fund after all other benefits and certain administrative costs have been paid, as well as how many valid claims are filed, the site says.
The parties await a hearing on November 14, 2025, at which time the court will decide whether to grant final approval to the terms of the Dignity Health class action settlement.
Court documents relay that, should the deal be ultimately approved, settlement payments and enrollment instructions for credit monitoring services will be issued to eligible class members within 30 days following the date the deal goes into effect or all claims have been processed, whichever is later.
