$1.09M Community Care Alliance Settlement Ends Class Action Lawsuit Over 2024 Data Breach

A $1.09M settlement ends a class action lawsuit against Community Care Alliance over a 2024 data breach.
A $1.09 million class action settlement will resolve litigation against Community Care Alliance (CCA) over a 2024 data breach that may have impacted the sensitive personal information of approximately 116,753 people.

The Community Care Alliance class action settlement received preliminary approval from the court on June 3, 2025 and covers anyone whose information was potentially compromised in the CCA data breach.

Community Care Alliance settlement class members who submit a timely, valid claim form will be able to receive up to $5,000 in reimbursement for documented monetary losses, two years of three-bureau credit monitoring and a pro-rata, or equal-share, cash payment from the remainder of the $1.09 million settlement fund.
Per the class action settlement website, documented monetary losses must be related to the CCA data breach and may include:
- Unreimbursed losses associated with actual fraud or identity theft;
- Out-of-pocket credit monitoring costs incurred between July 19, 2024 and the date the claim form is submitted; and
- Unreimbursed long-distance phone charges, gasoline for local travel, postage or bank fees.
Class members must submit reasonable proof and/or documentation with their claim form in order to obtain reimbursement for these losses, the website states.
The credit monitoring available through the Community Care Alliance settlement includes identity theft coverage for up to $1 million, fully managed identity recovery services and dark web monitoring.
The pro-rata cash payment is estimated to be approximately $100 per person, but may be increased or decreased based on the number of valid claims filed, the settlement website shares.
To submit a claim form online, settlement class members can visit this page and log in with the unique settlement claim ID found in their copy of the settlement notice.
Alternatively, a PDF of the claim form is available to print, fill out and mail back to the address listed at the top of the first page.
All claim forms must be submitted online or postmarked by October 1, 2025.
Additionally, Community Care Alliance has also agreed, as part of the settlement, to implement new cybersecurity measures and business practices to better safeguard the personal information stored on its systems.
A hearing is set for October 8, 2025 to determine whether the settlement will receive final court approval. Compensation will begin to be distributed to class members only after final approval has been granted and any appeals have been resolved.
The Community Care Alliance class action lawsuit claimed that a data breach on or about July 29, 2024 may have exposed the sensitive personal data, including Social Security numbers, names, addresses, personal customer data, credit cards and phone numbers, of thousands of individuals.
