A $1.5M Liberty Hospital class action settlement brings an end to litigation over an alleged 2023 data breach.
Liberty Hospital has agreed to pay $1,500,000 to resolve a class action lawsuit over an alleged December 2023 data breach that potentially exposed individuals’ private information to an unauthorized third party.

The Liberty Hospital class action settlement was preliminarily approved by the court on June 3, 2025 and covers all individuals in the United States whose personal information was potentially exposed to unauthorized third parties because of the data breach on or around December 19, 2023.
The court-approved website for the Liberty Hospital data breach settlement can be found at LibertyHospitalDataIncidentSettlement.com.
To submit a claim form online, settlement class members can go to this page of the settlement website and enter the unique ID and confirmation code found in their copy of the settlement notice. Class members can also download a PDF of the claim form to print, fill out and mail to the settlement administrator listed on the website.
All Liberty Hospital claim forms must be submitted online or postmarked by January 12, 2025.
Per the settlement website, Liberty Hospital settlement class members who submit a valid, timely claim form are eligible to receive up to $500 for documented expenses and losses related to the data breach. Reimbursable expenses include professional fees, costs associated with freezing or unfreezing credit, credit monitoring costs, and miscellaneous expenses such as postage or mileage. Claims for documented expenses and losses should include supporting documentation such as receipts or invoices, the settlement website says.
In lieu of a documented out-of-pocket-loss payment, class members can elect to receive an alternative cash payment of approximately $150. The alternative cash payment requires no proof and will increase or decrease on a pro rata (equal share) basis, depending on the total number of valid claims filed and the amount of funds available.
A final approval hearing for the Liberty Hospital class action settlement is scheduled for January 20, 2026. Compensation will begin to be distributed to class members only after final approval is granted and any appeals are resolved.
The Liberty Hospital class action lawsuit alleged that enormous amounts of personal information were compromised during the 2023 data breach. Specifically, the lawsuit alleged that the personal information exposed to third parties included names, addresses, dates of birth, medical records, medical treatment information, diagnoses, Social Security numbers, telephone numbers, health insurance information and email addresses.
