Modoc Medical Center, a hospital and healthcare services provider in California, has reported a data breach that may have exposed sensitive information.

According to a notice posted to its website, Modoc became aware of unauthorized access to certain files on its network on January 27, 2026. Following an investigation with cybersecurity specialists, the healthcare provider determined that an unauthorized actor accessed its systems and downloaded certain files between January 19 and 27.

A data review of the affected files revealed that the incident may have exposed names, Social Security numbers, driver's license or state identification numbers, financial account information, payment card information, passport numbers, military identification numbers, medical information and health insurance information. The information impacted varied by individual.

Modoc has started sending notices to potentially affected individuals by mail; a sample is shown below.

The Modoc Medical Center data breach was also reported to the Massachusetts Office of Consumer Affairs and Business Regulation on September 22, 2026.
