According to a notice posted to its website, NSE observed unauthorized access to its network on or around November 28, 2025. Following an investigation with cybersecurity professionals, the company determined on August 24, 2026 that certain files containing personal information may have been subject to unauthorized access and acquisition between November 6 and 29 of the year prior.

Information that may have been exposed in the NSE Insurance Agencies data breach, varying by individual, includes names, Social Security numbers, driver's license or state identification numbers, government identification numbers, financial account information, credit or debit card information, medical information and health insurance policy information.

NSE Insurance Agencies began mailing notices to individuals potentially impacted by the incident on September 21; a sample is shown below.
