Harmony Health Security Incident: What Happened?
Harmony Health Medical Clinic and Family Resource Center, a family practice medical clinic with four locations in northern California, has reported a recent data security incident involving a third-party vendor.
The healthcare provider was notified on December 12, 2025 about an incident experienced by TriZetto Provider Solutions, an insurance clearinghouse it uses to process insurance eligibility verification transactions. An investigation of unusual activity involving a client web portal determined that an unauthorized actor had accessed certain records in TriZetto’s systems between November 2024 and October 2, 2025.
The Harmony Health data breach may have impacted names, Social Security numbers, and contact information such as mailing and email addresses and telephone numbers.
Those affected by the Harmony Health Medical Clinic and Family Resource Center data breach are being notified by mail.
