Devereux Foundation Security Incident: What Happened?

The Devereux Foundation has reported a data breach potentially involving the information of certain current and former employees, clients, donors, payors and business partners of the multistate behavioral healthcare organization.

According to a notice posted to its website, Devereux detected suspicious activity on its network on November 9, 2025 and, after an investigation, determined that an unauthorized actor gained access to certain systems between November 6 and November 9, 2025.

Following a review to determine the nature of the affected data and which individuals might be impacted, Devereux concluded on June 23, 2026 that the breach potentially exposed names, dates of birth, Social Security numbers, driver's license or state identification numbers, U.S. Alien registration numbers, taxpayer identification numbers, digital/electronic signatures, financial account information, payment card information, medical information and health insurance information.

The Devereux Foundation data breach was also reported to the Vermont Attorney General's Office on July 23, 2026.

Before completing the review of affected data, the Devereux Foundation had reported the breach to the U.S. Department of Health and Human Services on January 8, 2026.
