Three Oaks Hospice Security Incident: What Happened?
Three Oaks Hospice, a Texas-based palliative and hospice care provider with locations in several states, has disclosed a data breach that may have exposed sensitive information.
According to a September 17, 2026 notice posted to its website, Three Oaks Hospice discovered unauthorized activity associated with its email environment on August 8, 2025. Following an investigation, the Dallas location determined on July 20, 2026 that unauthorized activity associated with its email system affected certain personal information.
Information that may have been affected during the Three Oaks Hospice data breach, varying by individual, included names, driver's license numbers, Social Security numbers, dates of birth, medical information and health insurance information.
Three Oaks Hospice notified affiliated hospices that were affected by the incident on August 17 of this year; the company is also in the process of notifying all individuals whose information may have been impacted.
The Three Oaks Hospice data breach was also reported to the Texas Attorney General's Office by individual hospice locations between September 17 and 21.
