SRCH learned on December 10, 2025 that an unauthorized individual had gained access to one of the systems used by TriZetto, a company that works with the health network’s electronic medical record system.
The Santa Rosa Community Health data breach may have compromised patient information such as names, Social Security numbers, dates of birth, contact details and certain health-related or insurance information.
Individuals impacted by the SRCH data breach are being notified by mail.
