A notice on PHMG’s website states that, on or around March 11, 2026, the healthcare organization learned of suspicious activity in its associate's hosted environment, which contained certain PHMG data. Following the discovery, PHMG alerted its business associate of the incident. An investigation determined that PHMG data was among the files accessed by an unauthorized actor during the Palomar Health Medical Group data breach, though PHMG’s own systems reportedly were not impacted.

The investigation is still underway, and the specific individuals and information affected by the PHMG data breach have not yet been identified. Potentially impacted data includes names, addresses, dates of birth, Social Security numbers, credit or debit card details, email addresses, usernames and passwords. The incident may have also compromised medical history details, health insurance information and medical record numbers, as well as data about diagnoses, prescriptions and more.

Individuals affected by the Palomar Health data breach will be notified as the investigation progresses.
