Optalis Health & Rehabilitation Data Breach: Lawsuit Investigation

Optalis Health & Rehabilitation Security Incident: What Happened?
Optalis Management Solutions, which provides post-hospital rehabilitation, skilled nursing and assisted living care under the Optalis Health & Rehabilitation and Cranberry Park Assisted Living names, has reported a data breach involving unauthorized network access.
According to a notice posted to Optalis’ website, the data breach occurred between April 14 and April 19, 2025. An investigation and document review, which ended on June 10, 2026, determined that some personal information was exfiltrated from Optalis’ system, including names, Social Security numbers, driver’s license or state ID numbers, credit or debit card information, financial account information, medical treatment and diagnosis information, and health insurance policy numbers.
Those potentially impacted by the Optalis Management Solutions data breach were notified beginning on or around June 29, 2026 (sample notice pictured below).
