According to a notice posted to its site, suspicious activity was detected in its network on or around May 20, 2025. A cybersecurity investigation revealed that an unauthorized individual accessed the network and obtained files containing sensitive patient information.
On December 2, 2025, the investigation concluded that the Nephrology Associates Medical Group data breach potentially affected protected health information, including names, Social Security numbers, dates of birth, medical information, treatment or diagnostic data, health insurance details, billing or payment information and credentialing data.
