Nephrology Associates, M.D., P.A., a kidney care company serving the Kansas City, Missouri area, has reported a data breach involving sensitive information.

According to a notice posted to its website, the healthcare provider became aware of unauthorized access to its network that had occurred between January 17, 2026 and April 9, 2026.

Following an investigation with cybersecurity professionals, Nephrology Associates determined on July 1, 2026 that certain information was acquired from its network during the incident, including names, Social Security numbers, dates of birth, driver's license or state identification number, government identification number, treatment/diagnosis information and health insurance policy information.

Nephrology Associates began sending notice of the incident to affected individuals on July 30, 2026, a sample of which can be seen below.

The total number of individuals affected by the Nephrology Associates data breach has not been publicly disclosed.
