Mosaic Life Care Data Breach: Lawsuit Investigation

Mosaic Life Care, Oracle Health Security Incident: What Happened?
On June 27, 2025, Heartland Regional Medical Center, which does business as Mosaic Life Care, reported a hacking/IT incident affecting 145,269 individuals to the U.S. Department of Health and Human Services.
Mosaic Life Care, a healthcare system serving 35 counties throughout Missouri, Kansas, Nebraska and Iowa, said it learned of the incident after an unknown party claimed to be in possession of patient information. After verifying the claim on April 29, 2025, Mosaic Life Care traced the data breach to Oracle Health/Cerner's systems in relation to data migration services it was providing.
On May 2, 2025, Oracle Health/Cerner confirmed that the files originated from its environment, revealing that access had been obtained using compromised credentials at least as early as January 22, 2025. Details of affected Mosaic patients were provided on June 6, 2025. The files involved in the data breach affecting Mosaic Life Care may include patients' names, Social Security numbers, driver’s license numbers, birthdates, treating physicians, service dates, medication details, insurance information, and diagnostic and treatment information, varying by individual.
Mosaic Life Care mailed patient notification letters on June 27, 2025 and noted that the data breach did not impact its own systems.
