According to an online notice, Lake Region Healthcare first discovered unauthorized access on or about May 19, 2025 and subsequently launched an investigation aided by cybersecurity experts. The investigation, concluding on June 5, 2026, determined that names, dates of birth, Social Security numbers, medical record numbers, patient account numbers, health insurance information, contact information, medical or treatment information, government-issued ID data, and financial information may have been involved, varying from person to person.
Those potentially impacted by the Lake Region Healthcare data breach are being notified by mail (sample notice pictured below).
