Horizon Healthcare RCM Data Breach: Lawsuit Investigation

Horizon Healthcare RCM Security Incident: What Happened?
Horizon Healthcare RCM, an Indiana-based company specializing in revenue-cycle management for hospitals, health systems and medical centers, recently announced a data breach that may have impacted individuals receiving services from its clients. The incident came to light on December 27, 2024, when a computer virus blocked access to certain network files.
An investigation revealed potential unauthorized copying of files between December 26 and 27. A review, completed on May 20, 2025, found that the most common data involved included internal Horizon numbers, customer numbers or other patient identifiers, along with general health insurance claims processing information. In some cases, a medical record number was also affected.
A smaller number of instances involved sensitive data like non-address contact details, birth dates, Social Security numbers, driver’s license and passport numbers, payment card data, and checking or financial account information. A report submitted to the U.S. Department of Health and Human Services states that 77,410 individuals are affected by the Horizon Healthcare RCM data breach.
Horizon Healthcare RCM is now notifying affected patients for whom it has contact information.
