According to a notice posted to its website, on January 15, 2026, while investigating an unsuccessful phishing attempt, Heart Care Centers of Illinois discovered historical suspicious account activity in connection with an employee email account.
Following an investigation with third-party forensics specialists, the company determined on June 11, 2026 that the unauthorized actor potentially accessed certain protected information of its patients and employees, including names, mailing addresses, Social Security numbers, dates of birth, driver’s license and state identification numbers, payment card information, financial account information and passport numbers. Additionally, the Heart Care Centers of Illinois data breach may have also affected medical information, such as treatments, conditions and diagnoses, prescription information, health insurance information, provider information and telephone and fax numbers. Information affected varied by individual.
The company began mailing notification letters to potentially affected individuals on July 10, 2026, the total number of which has not been publicly disclosed.
