Family Health Centers of Southern Indiana (FHCSI), operating multiple locations and a mobile dental unit, has reported a data breach that may have involved unauthorized access to sensitive information.
In a June 22, 2026 notice on its website, FHCSI disclosed that it first discovered unauthorized network activity on or around January 16 of the same year. Preliminary investigation found that, during the Family Health Centers of Southern Indiana data breach, an unauthorized individual may have accessed certain files. A subsequent review of the potentially impacted files determined that names, Social Security numbers, dates of birth, medical information, health insurance information, demographic information, and contact information may have been put at risk of unauthorized access.
Prior to FHCSI’s confirmation of the data security incident, reports of a possible FHCSI data breach emerged on dark web reporting sites. A February 3, 2026 post on dark web intelligence platform Breachsense indicated that ransomware group Termite had claimed responsibility for exfiltrating 250 GB of information from FHCSI’s system.
On June 18, 2026, once the investigation concluded, FHCSI began notifying potentially affected individuals by mail.
