Couve Healthcare Consulting, which does business as Evergreen Healthcare Group, has announced a data breach that may have exposed sensitive personal information. According to a notice posted to its website, Evergreen Healthcare Group discovered unauthorized activity on its cloud-based healthcare platform on December 3, 2025. The company activated its incident response plan and initiated an investigation, which revealed that specific files might have been accessed without authorization.
After a review identified affected individuals and catalogued the compromised data, the investigation concluded that information such as names, Social Security numbers, dates of birth, and medical data could have been accessed. Individuals affected by the Evergreen Healthcare data breach are being notified by mail. The company reported the breach to the Massachusetts Office of Consumer Affairs and Business Regulation on February 24, 2026.
Evergreen Healthcare Group operates over 50 nursing home communities across seven states.
