Doctor's Choice Home Care & Hospice, a home health and hospice provider in East Texas, has reported a data breach involving personal and health information.
According to a notice posted to its website, an unauthorized party accessed a clinical user account within the electronic medical record system provided by WellSky, a healthcare technology and software solution company, that Doctor's Choice Home Care & Hospice utilized. This unauthorized access to the WellSky account occurred between June 5 and July 24, 2026 via compromised credentials.
Doctor's Choice Home Care & Hospice identified the breach on July 21, 2026. The company, investigating with WellSky, confirmed that this account was the only point of unauthorized access and that the incident was isolated to the WellSky platform.
Information that may have been impacted during the incident included demographic information, Social Security numbers, scheduling information and clinical/treatment information.
The Doctor's Choice Home Care & Hospice data breach was also reported to the Texas Attorney General's Office on September 21, which noted that the breach affected 14,333 Texans and may have also exposed names, addresses and health insurance information.
