According to a letter from the company’s attorneys, individuals’ names and Social Security numbers may have been exposed as a result of the Daniel H. Cook Associates data breach.
The company began mailing notice letters to those affected by the Daniel H. Cook Associates, Inc. data breach beginning on January 16, 2026.
Daniel H. Cook Associates specializes in self-funded benefits administration and works in large part with labor organizations and their members.
