Cookeville Regional Medical Center in Cookeville, Tennessee, has reported a data breach impacting 337,917 individuals.
A notice on Cookeville Regional's website states that on July 14, 2025, the hospital detected unusual activity within its computer network. Partnering with a forensic security firm, Cookeville Regional confirmed that an unauthorized party had accessed and possibly acquired certain files between July 11 and July 14 of that year.
The investigation, concluded on March 16, 2026, determined that the files affected in the Cookeville Regional Medical Center data breach contained names, addresses, dates of birth, Social Security numbers, driver’s license numbers, financial account details, medical treatment information, medical record numbers, and health insurance policy details.
Cookeville Regional is sending notification letters to affected individuals (see sample notice below).
