A notice on Cookeville Regional's website states that on July 14, 2025, the hospital detected unusual activity within its computer network. Partnering with a forensic security firm, Cookeville Regional confirmed that an unauthorized party accessed and possibly acquired certain files between July 11 and July 14, 2025.

The investigation, concluding on March 16, 2026, determined that the files affected in the Cookeville Regional Medical Center data breach contain names, addresses, dates of birth, Social Security numbers, driver’s license numbers, financial account details, medical treatment information, medical record numbers, and health insurance policy details.

Cookeville Regional is sending notification letters to affected individuals (see sample notice below).
